We provide comprehensive wound and ostomy consultation services designed to support long-term care homes through a combination of in-person clinical expertise, continuous virtual support, and targeted education.
In-Person Clinical Rounds
Our clinicians conduct in-person wound rounds every six weeks to assess complex and non-healing wounds, identify barriers to healing, and develop individualized, evidence-based treatment plans. Where clinically appropriate and when healing potential is present, we provide in-home conservative sharp wound debridement. During rounds, we offer bedside teaching and mentorship to the wound care lead, strengthening clinical decision-making and continuity of care.
Education and In-Service Training
We deliver structured in-service education for frontline staff, with a strong focus on wound prevention, early risk identification, and best practice wound and ostomy management. Education is tailored to the needs of each home and supports sustainable practice change.
24/7 Virtual Consultation Support
Our partner homes have access to 24/7 virtual consultation services, providing timely clinical guidance for wound deterioration, new admissions with wounds or ostomies, and urgent clinical concerns. This ensures early intervention, reduces avoidable complications, and supports confident decision-making at the point of care.
Specialized Education and Clinical Support
We also offer virtual or in-person education on specialized topics, including PICC line care, nephrostomy care, and other wound- and ostomy-related clinical topics, based on the needs of the home.
Our integrated model of scheduled in-person rounds combined with continuous virtual support has demonstrated significant positive clinical outcomes across our partner long-term care homes, improving healing outcomes, enhancing staff confidence, and reducing avoidable hospital transfers.
